Look at the Attempting call From: *line, that's the internal IP of the gateway machine, so it looks like
that is correct.
The RTP media target: 188.165.199.37:45536 corresponds to www5.pbxes.com which it finds all by
itself as a consequence of the localphone trunk being registered there. Clever stuff *(well I think so).
My gateway seems to be called skypests and not gateway, probably *not important
